Parental Permission Form

For when:

Your minor is 17 years old AND will be attending the convention without a parent or guardian present
(must be notarized)

Your minor is of any age younger than 18 AND you will be attending the event with them
 

Temporary Guardian Form

For when:

Your minor is of any age younger than 18 and will be supervised by an adult who is NOT their parent or guardian (must be notarized)
